Double-hit lymphoma (DHL) is an aggressive type of cancer that develops from B lymphocytes (B cells). It's sometimes considered a rare subtype of diffuse large B-cell lymphoma (DLBCL), affecting about 5 percent of people with DLBCL.
DHL gets its name from genetic mutations involving two specific genes. The cancer is caused by translocations (when pieces of chromosomes break off and combine abnormally) involving the MYC gene and the BCL2 gene. Less commonly, the BCL6 gene is involved instead of BCL2.
These genetic rearrangements cause problems with cell growth and death. BCL2 normally helps control cell death, while MYC promotes cell multiplication. When both genes are rearranged, there's too much cell division and not enough cell death, resulting in cancer.
DHL primarily affects older adults and is slightly more common in males. It's a very aggressive disease that doesn't respond well to conventional treatments, and people with this type of lymphoma tend to have a poorer prognosis.
