Is nausea, dizziness normal?

My husband has st4 MCL and was diagnosed in September last year. He had 6 rounds of chemo; Rituximab & Bendamustine. The finished in feb and then onto maintenance which didn’t work. He’s on 2nd month of Brunkinsa and was absolutely ok for the 1st mth. For the last week he’s been feeling sick, dizzy when standing, can’t eat because it makes him feel so poorly. Yes this is normal. I am so sorry to hear about your husband and his symptoms of nausea and dizziness. I have had the same symptoms and have had the same treatment he had. My oncologist gave me the he odanesterom and he also gave me Prochlorper. If I don’t have relief from the odanestrom then I take the other one. They are safe to take together but it has to be taken separately. Let the oncologist know what is happening and ask him if he thinks the one that I gave you is going to help him and if so have him give you that one too. I hope everything goes well. What a lot you're both navigating right now — it's completely understandable to feel alarmed when things change so suddenly. To answer your question: yes, it can be normal for side effects to appear or worsen after the first month on Brukinsa (zanubrutinib), even if the first month felt fine. Side effects don't always show up immediately.

Known side effects of Brukinsa include:

- Diarrhea
- Bruising
- Cough
- Anemia
- Neutropenia (low white blood cell counts)

The dizziness when standing is also a known concern with some lymphoma treatments, as it can relate to drops in blood pressure when changing position, which can risk falls — so that's worth flagging urgently. The nausea could have several contributing factors beyond just Brukinsa, including anxiety, pain, inflammation, stomach irritation, or even constipation. The pain medications themselves can also cause nausea and dizziness, so reducing them (as you've done) was a sensible instinct.

Since ondansetron isn't working, his oncology team may be able to suggest alternative anti-nausea options — it's really worth contacting them directly rather than just the GP, as they'll have more specific knowledge of his treatment.

Please don't delay reaching out to his oncologist about:
- The sudden change in how he's feeling
- The dizziness when standing (fall risk)
- The inability to eat
- The leg and stomach pain

These symptoms together deserve specialist eyes on them quickly. You're clearly doing a brilliant job advocating for him. 💙

Thank you all for you kind thoughts. Back on an even keel once more. We stopped 1 morphine tab in the morning, and went back to when he was feeling better. Ended up in hosp for a day and he had bloods and fluids. Then a different anti sick (metaclopramide) for a week and much improvement.

Elsa I also have MCL diagnosed Sept 2023. I just finished my 21st cycle of 24 ,the same B&R then Brukinsa every day .Other than some major fatigue I am not experiencing any other major issues. I have been on an exercise program and stretching program every day . I suggest for him to try and stay active and think positive. Prayers going his way
