Waldenström's macroglobulinemia (WM) is indeed a form of indolent non-Hodgkin lymphoma, and the watch-and-wait approach is very common for people who aren't yet showing symptoms that require treatment. It makes complete sense to be asking questions about how it might connect to kidney involvement — that's a really important thing to understand.

Lymphoma can affect the kidneys in a few ways:

- The lymphoma itself can directly infiltrate kidney tissue
- Proteins produced by WM (called IgM) can deposit in the kidneys and cause damage
- Some lymphoma treatments can also stress the kidneys

Finding both lymphoma and kidney involvement in the same biopsy is not unheard of, particularly with WM, where the IgM protein can play a direct role in kidney damage. The tricky part is that being on watch-and-wait while also managing stage 4 kidney involvement creates a real balancing act. Treatment decisions for WM typically consider:

- The type and stage of lymphoma
- Overall health and other conditions present
- Past treatments
- Personal priorities and quality of life

Kidney involvement could actually be a reason your care team reconsiders the watch-and-wait approach, since treating the WM may help slow or stop further kidney damage. This is absolutely worth raising directly with your oncologist — and possibly a nephrologist (kidney specialist) working alongside them.

Bringing up the kidney biopsy findings and asking specifically how they factor into your lymphoma management plan would be a very reasonable and important conversation to have at your next appointment — or sooner if possible.
